Best way to pour a concrete pad for a shed?

I'm getting ready to set up a shed in my backyard and want to make sure I do the foundation right. I've decided on a concrete pad, but I'm unsure about the best way to go about it. I want something durable that won't crack or shift over time. Some people say rebar is necessary, while others suggest wire mesh or just a solid pour. I also want to make sure the ground is prepped properly before pouring. If anyone has experience with this, I'd love to hear how you did yours and any tips to avoid mistakes. Any advice would be appreciated, thanks
 
I'd say rebar is not needed, but crush the stone down before pouring the concrete down. don't forget to remove the grass (if you have grass) and dig it down just a little deep for the concrete
